Any contact information you provide on our “request an appointment” page will be used only to contact you to set up the requested appointment. No e-mail addresses or other contact information, will be sold or shared with any outside party. The only people who will see the information you provide will be our staff and the people who service our website.

If you ever wish to change any information you provide us, simply call the office.
